Get scheduled within two weeks by completing the steps below:

1. Read through our website and be sure to contact admin@mesaguidance.com with any questions.

2. Complete the New Patient Registration form located here

3. Wait for a confirmation email with a list of available dates within 2 weeks. You will receive a confirmation text and email once you're scheduled.

4. Complete the Electronic Intake Form

5. Review and sign the CC Auth and Informed consent contracts emailed to you

6. Send any lab work, previous records or other documents you would like the doctor to have to admin@mesaguidance.com to be added to your health records before your visit.

7. The day of your appointment, log on to the Mesa Guidance Psychiatry virtual office 10 minutes early to ensure your connection is working and you're comfortable. If you're running late, please text/call the main number 970-661-3555.

After your appointment, your labwork will be mailed to you to take into your local lab for any additional recommended testing. Your scripts will be called in the day of your appointment and will be ready whenever your pharmacy completes the order.
